Cytokeratins (CKs, also called keratins) are part of the cytoskeleton family of intermediate filament-forming proteins (IFPs) that provide mechanical support and also provide a role in cell differentiation and tissue specialization to maintain cellular integrity. Cytokeratins are expressed as pairs in both keratinized and non-keratinized epithelial tissue, where they constitute up to 85 % of mature keratinocytes in the vertebrate epidermis.Two types of Cytokeratins are known to exist, namely acidic type I (Cytokeratins 9-23) and basic type II (Cytokeratins 1-8). These Cytokeratins make homo and heterodimers that are specific for particular type of carcinoma. Motifs in certain Cytokeratins make them likely substrates for caspase degradation, and their subsequent release occurs during the intermediate events in apoptosis. Cytokeratin 7 which is a type II keratin (also known as sarcolectin) is one of the 20 different Cytokeratins identified and is expressed in the simple epithelia lining the cavities of the internal organs and in the gland ducts and blood vessels. The genes encoding the type II Cytokeratins are clustered in a region of chromosome 12q12-q13. Alternative splicing may result in several transcript variants and antibodies to cytokeratin 7 are useful in the differential diagnosis of prostate carcinomas where CK7 is rarely expressed. Cytokeratin 7 also inhibits the synthesis of interferon-dependent secondary proteins thus reversing the antiviral effect of interferon induction and restoring cells to their status ad primum. In normal and transformed cells, Cytokeratin 7 localizes to the membrane. Expression of CK 7 is seen in majority of cases of cervical squamous cell carcinomas and is negative in other cases of carcinomas specially arising from the lung and GI tract.
